Patient Intake Forms

Please click on the appropriate forms below that Dr. Stephanie has asked you to complete. All Health History Forms will be automatically submitted to Barto Family Chiropractic upon your clicking the “Submit Form” button at the bottom of that page. All other forms will require you to print them at home, manually complete them with a pen, and bring them with you to your appointment. Completing these forms prior to your visit allows Dr. Stephanie to better understand your health and gives you the benefit of a more time-efficient experience at the office.
